Annual Open Enrollment
Open enrollment only happens once a year, so make it count! During this time, you can:
- Enroll in any insurance plan: If you don’t have coverage, this is your chance to get insured.
- Change your current plan: You can switch to a different coverage level that better suits your needs.
- Cancel your plan: If you no longer need coverage, you can opt-out.

ID cards will be mailed in December. You can discard your UMR ID card 1/1/26.

FLEXIBLE SPENDING ACCOUNTS (FSA)
Provider: P&A Group
Enrollment forms are due on or before December 15, 2025, for all employees.
Health FSA contribution limit: $3,400
A health care FSA may be used to pay for any out-of-pocket expenses connected with health/dental/vision care for you, your spouse, or any eligible dependent as defined by the IRS. Examples of eligible expenses may include health plan deductibles, copays, eye exams, and more.
Dependent Care contribution limit: $7,500
The dependent care FSA allows for tax-free reimbursement of eligible care for a qualified individual. Expenses claimed must be for care required so that an eligible employee (and spouse, if applicable) can work, look for work, or attend school full-time.
Qualified individuals include dependent children under the age of 13 claimed on your federal tax return, a spouse, or other tax dependent who is physically or mentally incapable of self-care.
FREE LIFE INSURANCE and AD&D
Provider: Lincoln Financial Group (LFG)
LL Pelling provides you, your spouse, and eligible dependent children with free term-life insurance and accidental death and dismemberment policies.
Life insurance provides financial security for the people who depend on you. Your beneficiary(ies) will receive a lump-sum payment of $50,000 if you die while employed. You will receive a lump-sum payment of $20,000 if your spouse dies and $10,000 if your child dies while you are employed.
Benefits will reduce at ages 65, 70, 75, and 80.
Accidental Death & Dismemberment (AD&D) insurance provides a payment to you or your beneficiary(ies) if you die or are seriously injured in an accident. LL Pelling provides AD&D coverage at no cost to you in addition to the company-paid life insurance.
VOLUNTARY LIFE INSURANCE
Provider: Lincoln Financial Group (LFG)
During open enrollment, you may:
- Purchase insurance for the first time
- Increase your current levels of coverage
Coverage Limits:
- Up to $40,000 for yourself. Coverage exceeding $300,000 requires a health questionnaire.
- Up to $20,000 for your spouse. Coverage exceeding $30,000 requires a health questionnaire.
- Up to $10,000 for your child(ren). Coverage cannot exceed $10,000.
No action is needed if you are not making a change during open enrollment.
VOLUNTARY ACCIDENT & CRITICAL ILLNESS
Provider: Lincoln Financial Group (LFG)
Ask human resources for an enrollment form.
VOLUNTARY LONG-TERM DISABILITY
Provider: Lincoln Financial Group (LFG).
- Premiums are based on the employee’s annual earnings for the prior year.
- Complete an enrollment/waiver form to add or drop coverage.
SHORT-TERM DISABILITY
The weekly benefit amount is $600 per week.
Employees who pay the $1/month premium will not be taxed on any STD benefits received.
Employees may choose to not pay the premium; they would be taxed on any STD benefits received.
Any employee receiving short-term disability benefits will have their insurance premiums deducted from the $600 weekly benefit.
401(k) RETIREMENT
Administered by Fidelity Investments.
New hires are automatically enrolled for 5% contribution unless they opt out.
Matching Contributions
End of Year: Annually, the company will contribute 2.5% of your total wages to a traditional (pretax) 401(k) account whether you participate in the 401(k) or not. The contribution is made after year-end, and employment through December 31st is required.
Weekly: The company will match 50% of your contribution up to 5% of your gross weekly earnings. If you contribute 5% of your weekly paycheck, you will receive an additional 2.5% contribution from the company.

Frequently Asked Questions
I'm a seasonal employee. When's my Open Enrollment?
Your Open Enrollment starts on your first day back from layoff in 2026 for 30 days.
To participate in Flexible Spending Accounts, you must enroll by December 15, 2025. No exceptions.
How do I enroll, change, or waive a benefit?
To request an enrollment form, contact Carrie or Stephanie in Human Resources. They can send you a copy via email, mail, or provide one to you in person.
Make sure to submit your enrollment forms and supporting documents before your open enrollment period ends!
What happens if I do nothing during open enrollment?
If no action is taken during this open enrollment period, your 2025 benefit elections will automatically carry over, except for your FSA health care and dependent care elections. The IRS mandates that employees make an active election yearly; therefore, you must re-enroll by December 15, 2025 to participate in 2026.
Can I enroll in a Flexible Spending Account (FSA) if I don't have health insurance?
You do not need to be enrolled in the company health insurance, dental, or vision plan in order to participate in the Flexible Spending Account.
Health FSA and Dependent Care FSA enrollment forms are due by December 15, 2025.
How soon can I use my FSA account?
Your health spending account funds are available to you on Jan. 1, 2026. Funds are available regardless of how much you’ve contributed due to the “uniform coverage rule”. This applies to all employees, including seasonal employees who have not yet received a paycheck.
You can start using your dependent care FSA benefits as soon as money is deposited into your account from your paycheck. Unlike a health spending account, you can’t use the full amount of your election right away, only the funds as they are contributed throughout the plan year.
What family members can I add to my benefits?
Legal spouses and natural, step-, or adopted children up to their 26th birthday, and other qualified dependents, such as disabled children age 26 and older, with required documentation. It also includes any child for whom you are required to provide coverage under a Qualified Medical Child Support Order (QMCSO). Eligibility documentation (e.g., marriage or birth certificate) is required for new enrollees or to add a dependent.
Can I add my partner? We've been together for years.
Unfortunately, only legal spouses with marriage certificates can be added to any benefits offered. Our plans do not recognize common law marriages. To add a spouse, you must provide an official marriage certificate as supporting documentation.
If I change my mind after Open Enrollment, can I change my benefits?
No, you cannot change your 2026 benefit elections after the end of your Open Enrollment period, unless you have a qualifying life event.
Please think carefully about your choices, as changes and corrections are only allowed during Open Enrollment or within 30 days of a qualifying life event.
